The GT Identification System currently has three customizable surveys: 

  • The Referral Survey 
  • The Parent Survey
  • The Teacher/Collaborator Survey 

To customize any of these surveys, follow the steps below: 

  1. Click on the "Settings" menu option on the GT Identification System homepage. 
  2. Scroll down to the section titled "Survey Questions".
  3. In the "Selected Survey" section, pick the survey you would like to customize. 
  4. Edit the title, answer type, and instructions, if desired. Note: The GT Identification System currently only allows one type of survey question, checkboxes or radio buttons. 

To add a new question to the survey: 

  1. Scroll down to the bottom of the question list and type in the new question in the text field titled "New English Questions Text". If you'd like to add the same question in Spanish, do the same in the text field below in the "New Spanish Questions Text" section. Note: Any text added to the GT ID System is not automatically translated into Spanish.
  2. Click the blue plus (+) button next to your question to add it to the survey.
  3. Click the blue "Save Custom Survey" button. 


To remove  an existing question from the survey: 

  1. Click the red minus (-) button next to the question you would like to remove. 
  2. Click the blue "Save Custom Survey" button. 


If you would like to revert a survey back to its original unedited form, just click the yellow "Reset to Default Survey" button.
